Lenawee Community Mental Health Authority of Adrian MI is looking to hire a full-time Wraparound Facilitator .

Our agency provides or manages a full array of services to adults and children with mental health issues, emotional disturbances, substance use disorders, and developmental disabilities. A DAY IN THE LIFE OF A WRAPAROUND FACILITATOR As a Wraparound Facilitator, you assist the families of referred youth in navigating systems and better meeting their needs. You understand that traditional services often are not effective for this population and look for opportunities to increase access to additional supports. You find the gaps in support and help bring about improvement as you connect with multiple community partners and systems. Often, by the time you get involved, other systems or the family feel like giving up. Your job is to instill hope and reenergize the efforts.
You assist families in identifying supports, building a team, designing a strength-based plan, and facilitating team meetings. The plan of service that you prepare is based upon discoveries about strengths and culture, a needs assessment, a crisis and safety support plan, a wraparound plan, outcomes, and the development of a family mission statement. You link and coordinate services and inter-agency resources. You may need to create and facilitate services or supports, which do not presently exist in order to truly meet their needs.
You monitor the progress that providers are making toward outcomes and facilitate the development of transition strategies. With careful attention, you manage funds allocated through the Community Team and provide them with the data required for determining consumer satisfaction, progress toward measurable outcomes, and efficient utilization of resources. You will find creating a path to resilience, recovery, wellness, and self-determination for the youth that you work with to be very rewarding!
QUALIFICATIONS FOR A WRAPAROUND FACILITATOR Bachelor's degree from an accredited program in Social Work is preferred; or a Bachelor's degree from an accredited program in a human services field ; Licensed by the State of Michigan to practice as a social worker (if you have a BSW) ; Two- years of post-graduate experience working with children and their families; Basic computer knowledge, including experience with Microsoft Word, Excel, and PowerPoint; Valid driver's license. The annualized salary for this position is $43,758 per year (based on a 37-1/2 hour work week).
The benefits package includes: Employees receive a generous paid leave benefit, and paid leave begins to accrue on an employee's first day of employment; an employee would receive about 22-1/2 paid days off in the first year. The number of paid leave hours an employee accrues is based on length of service; the paid leave time accruing increases after one year, five years, and ten years of employment .
Employees receive scheduled annual salary increases, starting after one year of employment.
Paid Holidays -- LCMHA observes 13 paid holidays per year per year; employees are eligible for paid holidays as of the first day of employment.
Employees are eligible to enroll in comprehensive medical, dental, and vision insurance as o f the first day of employment.
Medical insurance: the agency covers: 100% of the cost of medical insurance for two of the plan choices and 98.5% of the cost of medical insurance for the third plan choice (for employees who enroll at the employee-only coverage level). At least 86% of the cost of medical insurance for employees who enroll at the employee-plus-one or family coverage level (depending on the plan selection). Vision insurance: the agency covers 87% of the cost for all coverage levels; Dental insurance: the agency covers 100% of the cost for all coverage levels. Employees who have medical insurance coverage elsewhere receive a medical insurance opt-out payment (as of the first day of employment).
Long-Term Disability and Life Insurance (at no cost) -- coverage starts on the 91st day of employment.
Retirement Benefits: participation starts as of the first day of employment.
WORK SCHEDULE The typical schedule is Monday - Friday, 8:30 AM - 5:00 PM with a one-hour unpaid lunch. He/she would likely need to schedule occasional appointments with clients and/or their families in the evenings, but would be offered a flexible work schedule to make up for these types of after-hour work.
