Details of the offer

Compensation:
Hourly pay ranging from $37 to $40, commensurate with experience
Full-time employees are eligible for benefits after 90 days, including a 401(k) with company match after 1 year of service
Qualifications:
Active Compact Multi-State RN License
Previous experience in Care/Case Management, particularly in telephonic settings
Registered Nurse with over 5 years of clinical RN experience
Proficiency in computer usage and documentation
Comfortable with contract durations of 60-90 days or more
Experience with Medicare/Medicaid/Managed care plans is advantageous
Responsibilities:
Engage in remote telephonic case management, adhering to the model of care to support members in managing their healthcare needs
Perform initial assessments and medication reviews, collaborating with members to develop personalized care plans and connect them with suitable resources
Liaise with the member's care team, community services, and vendors to ensure comprehensive support
Conduct regular follow-ups with members, at least every 30 days, to assess progress, address concerns, and provide necessary information or services
Educate members on their health plan coverage and assist in managing identified health conditions
Performance Expectations/Metrics:
Maintain a caseload of 75 actively managed members
Achieve an audit score of 90% or higher
Ensure case duration of 90 days or as required
