Responsibilities
• Assisting with the delivery of community support services for individuals receiving services through the Clustered Apartments Program• Providing training to clients in daily living skills• Facilitating development and/or enhancement of personal support systems within the client's community• Assisting clients in creating and participating in recreational activities• Performing bimonthly apartment inspections and providing input/skills training on household management• Maintaining work productivity standards• Documenting according to clinical policies and procedures• Maintaining compliance with all policy and procedures• Working cooperatively with other departments and members of the community• Participating in all required meetings
Requirements
• HS Diploma/HSE/GED is required• Two years of mental health or direct health care experience preferred• Reliable transportation, valid driver's license, and proof of continuous auto insurance required• Knowledge of DMH, CARF, and federal or state program regulations preferred
