Responsibilities
• Assesses and evaluates patient needs in developing plan of care• Provides education regarding diabetes and treatment options• Updates care plan as needed within scope of practice• Acts as a resource for health care providers, patients, and community• Serves as an educator and resource to medical and hospital staff• Maintains communication with physicians and other health care providers• Assesses the educational needs and learning barriers of designated population• Integrates health promotion and preventive aspects of care• Participates in community events and coordinates support group meetings• Participates in the development and evaluation of patient education program and materials
Requirements
• Registered Nurse license or Dietitian certification• Dietitian (RD) registration with the Academy of Nutrition and Dietetics• Diabetes Educator (CDE) certification• Bachelor's Degree in Nursing or Dietetics• 2 years of experience in a registered nurse or dietitian role with advanced clinical and technical knowledge of Diabetes• Maintain credentialing and continuing education requirements for recognition and representation of accredited DSMT program• Excellent interpersonal and group communication skills• Ability to effectively communicate with patients, public, medical facility staff, and physicians
