Job summary
The RHC Quality RN works to ensure that the environment and delivery of care meet or exceed standards in the Rural Health Clinic.Helps achieve quality and patient safety priorities through quality assurance and improvement.Coordinates performance management and quality improvement for clinic employees.
Responsibilities
• Build, direct, manage, and ensure implementation and effectiveness of the CMS Performance Management and Improvement Services.• Ensure rural health clinics meet or exceed current standards of Rural Health Clinic Conditions of Certification.• Research and design clinic performance and quality training.• Oversee HEDIS measures and Chronic Care management.• Improve quality gap closures and transitional care management.• Monitor primary care prevention and screening services.• Ensure efficient referrals to specialists and manage population health.• Monitor patient risk scores and ensure timely prior authorizations.• Promote an organizational culture of safety reporting and address safety issues.• Participate, educate, and assist in other projects as required.• Assist Clinic Director with administrative duties.• Supervise clinics in the absence of Clinic Director.
Requirements
• High school graduate or equivalent.• Associate or bachelor's Degree in nursing.• Master's Degree preferred.• 10 years of experience as a Registered Nurse, preferably in a clinic setting.• 5 years of health quality improvement or risk management experience.• 5 years of nurse management or Case Management experience.• 5 years of quality management experience.• Current state license as a Registered Nurse with the Louisiana Board of Nursing.• BLS certification.• Strong written and verbal communication skills.
