Responsibilities
• Perform initial and on-going assessment of patients• Assist in developing and implementing the Hospice Plan of Care• Educate patients and family on Plan of Care• Act as primary liaison between physician, patient/family, and hospice team• Maintain accurate and updated medical records• Make routine supervisory visits• Participate in agency on-call schedule• Abide by company policies and procedures• Promptly report any suspected violation of compliance standards
Requirements
• Registered Nurse (RN) license required in State of Louisiana• Minimum of two (2) years full-time experience as an RN• Experience in hospice care preferred• Current CPR certification required• Knowledge of EMR is required• Previous Hospice experience preferred• Compassionate RN with great time management skills
