As an integral part of our primary care facility, the Registered Nurse will play a vital role in providing exceptional patient-centered care.
Key Responsibilities:
Collaborate closely with team members to ensure a seamless and patient-focused approach to care delivery.
Conduct thorough assessments of patients' physical and psychosocial status through interviews, health histories, and physical examinations.
Facilitate continuity of care for patients, working efficiently and effectively with the healthcare team.
Provide comprehensive health education to patients, including information on treatment options, prescribed therapies, and medication expectations.
Maintain accurate and concise documentation of patient interactions and treatment plans.
Administer medications, treatments, immunizations, and injections as prescribed by healthcare providers.
Assist with patient medication refills and ensure compliance with regulatory requirements.
Support patient education efforts to increase understanding of disease processes, terminology, and treatment options.
Collaborate with other healthcare disciplines to ensure continuity of care and optimal patient outcomes.
Actively participate in Quality Improvement activities to enhance patient service delivery and clinical outcomes.
Make appropriate referrals to other healthcare providers and consult with specialists as needed.
Perform other duties as assigned.
Qualifications:
Current license to practice as a Registered Nurse in New York State, with BCLS certification.
Minimum of one (1) year of experience in a medical office setting, preferably in HIV/AIDS or other infectious disease practice.
Demonstrated phlebotomy skills are preferred.
Excellent written and verbal communication skills.
Proficiency with computers and MS software (Outlook, Word) preferred.
Familiarity with electronic charting systems (e.g., Medent) preferred.
Sensitivity toward cultural diversity, people living with HIV/AIDS, and lifestyle and addiction issues is essential.
