General Description As Quality Metric Coordinator, you will be responsible for ensuring accurate and timely reporting of healthcare quality metrics. This role involves analyzing and interpreting healthcare data, collaborating with cross-functional teams, implementing strategies to improve and maintain high-quality standards in healthcare reporting, and development of process improvement plans to reduce open care gaps. Essential Job Duties Conduct prospective chart preparation for clinical diagnostic and quality opportunities in advance of patient visit to ensure a plan to address open care gapsInteract with healthcare professionals to provide report-specific feedback and assist in resolving discrepanciesEngage with leadership and care team members to ensure patients are prioritized for scheduling based on outstanding care gapsContact patients to assist with medication adherence and preventative screening schedulingCollaborate with Accountable Care Organization (ACO) groups and various insurance providers to identify and close gaps in patient careCollaborate with cross-functional teams to gather, validate, and integrate data from various sourcesGenerate comprehensive reports to measure and track performance metrics related to healthcare quality measuresData entry of health plan quality metricsSupport and facilitate implementation of performance improvement activities resulting from identification of care gapsStay informed of changes in quality reporting requirements and implement necessary adjustmentsMaintain strict confidentiality and adhere to all HIPAA guidelines and regulationsPerform other related duties as assignedMinimum Qualifications One (1)+ years' experience within a healthcare settingKnowledge of clinical nursing practices and medical terminologyUnderstanding of reportable quality metricsOne (1)+ years' experience abstracting and utilizing information from electronic medical records, documents, and related informationProficiency in using Microsoft Office programs and web applicationsAbility to analyze data, share information and make recommendations for improvementsNumerical, analytical, and investigative skillsAbility to work in both independent and collaborative settingsDemonstrate time management skills and ability to adhere to project productivity metrics and time linesStrong interpersonal and communication skillsStrong customer service focusPreferred Qualifications Licensed Practical Nurse (LPN)Registered Nurse (RN)Continuous quality improvement experienceCare management experienceBilingual (English/Spanish)Healthcare Effectiveness Data and Information Set (HEDIS) quality reporting experienceOther quality metrics abstraction and reporting experiencePhysical Requirements / Working Conditions Must be able to sit for long periods of timeMust be able to stand, walk, stoop, bend, squat, and stretch for portions of the dayMust be able to lift and carry up to twenty (20) poundsMust be able to operate standard office equipment
