Job summary
The primary responsibility of the Physical Therapist (PT) is to plan, delegate and provide safe and competent physical therapy interventions as ordered by physician.Ensure the highest degree of quality care for the villagers at all times.

Responsibilities
• Provide physical therapy assessments and develop individualized treatment plans.• Supervise Physical Therapy Assistants, provisional licensees, students, and support personnel.• Identify the need for referrals to other disciplines to address comprehensive care needs.• Demonstrate competent technical skills according to Physical Therapy standards of practice.• Communicate changes in villager's condition.• Participate in discharge and community mobility planning.• Prescribe necessary durable medical equipment.• Provide accurate and timely billing records and supervise clinical documentation.• Respond to family requests and concerns.• Observe confidentiality and safeguards for patient information.
Requirements
• Current Colorado physical therapy practices and procedures.• Experience in providing care in post-acute or long-term care environment preferred.• Graduate of an accredited Physical Therapy Program.• Current, unencumbered, active Colorado Physical Therapy License.• CPR certification.• Valid, unrestricted Colorado driver's license.• COVID Vaccine or Qualifying Exemption Required.• Pass background, CAPs, and pose no direct threat to the health or safety of individuals in the workplace.
