Job Description:
We are seeking dedicated Medical Assistants to join our team and support patients with chronic conditions through our remote patient monitoring program. As a vital member of our team, you will play a crucial role in assisting patients in self-managing their chronic conditions through a collaborative process involving healthcare providers, our team, and specialized monitoring devices. These devices transmit medical data remotely, allowing you to regularly engage with patients to review their health status.
Key Responsibilities:
Stay informed of CMS regulations to ensure compliance.
Educate and motivate patients to participate in the program effectively.
Conduct group or individual training sessions for potential patients.
Document patient interactions clearly and concisely in the electronic medical record (EMR).
Enroll referred patients into the Remote Patient Monitoring Program.
Regularly review patient data and prioritize communication with patients based on their health status.
Educate patients on self-management skills and assist in goal setting for various chronic conditions.
Build and maintain positive relationships with internal and external stakeholders.
Act as a liaison between patients and physicians' offices.
Utilize motivational interviewing techniques to support patients in self-management.
Collaborate with patients to optimize the management of their chronic conditions.
Communicate patient needs effectively with providers and staff.
Ensure compliance with standards, policies, and procedures, and maintain accurate documentation.
Manage inventory of medical equipment.
Provide weekly reports on patient and platform results.
Possibility of travel between provider offices to support team members.
Perform other duties as required.
Minimum Qualifications:
Certified Medical Assistant (CMA) credential.
Minimum of two (2) years of clinical experience.
Proficiency in Microsoft 365, Teams, and electronic medical record systems (e.g., E-clinical, Athena, NextGen).
Ability to work independently with minimal supervision.
Strong critical thinking skills and attention to detail.
Demonstrated honesty, integrity, and professionalism.
Excellent time management skills.
Strong public speaking skills are a plus.
