Responsibilities:
Patient Management: Employ a proactive approach to oversee patient care, bridging gaps in both clinical and non-clinical services with a focus on resolution.
Resource Coordination: Identify and facilitate access to community resources that enhance patient care and well-being.
Patient Education: Empower patients with knowledge and tools to manage chronic conditions effectively, promoting health literacy and self-care.
Medication Management: Ensure medication adherence and safety through comprehensive management, including reconciliation, refill coordination, and addressing concerns.
Service Coordination: Facilitate timely delivery of essential services, such as Home Health, Durable Medical Equipment (DME), and Home Infusion, to meet patient needs effectively.
Patient Support: Foster open communication with patients, addressing inquiries, concerns, and needs to build trust and facilitate positive outcomes.
Referral Management: Assist in coordinating referrals and scheduling appointments, ensuring seamless transitions in patient care.
Additional Requirements:
Active Multi-State/Compact License (LPN) (NLC) (LVN)
Technical Aptitude: Proficiency in Microsoft Office Suite.
Excellent Communication Skills: Both written and verbal communication skills are essential for effective patient interaction.
