Interdisciplinary Team (IDT) Administrative Assistant Location Kalamazoo, MI (West Douglas area) : Interdisciplinary Team (IDT) Administrative Assistant Clinical Care Registered Nurse Who is P.A.C.E? The P rogram of A ll-Inclusive C are for the E lderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and - through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides - coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. Primary Purpose Under supervision of the Center Manager, the IDT Administrative Assistant will be responsible for supporting the IDT with administrative needs, quality and compliance, transition coordination, and tech coordination. Accountability Chart Key Roles Provides administrative support to the IDT Provides excellent customer service to Senior Care Partners P.A.C.E. (SCP) outside vendors Provides excellent customer service to SCP participants/caregivers Duties and Responsibilities Coordination and facilitation of transitions alongside of RN Transition Navigator. Main communicator with SCP outside vendors to include change forms, sending out referrals, coordinating facility required paperwork, communicating IDT plan to family and AFC. Mailing, faxing, and form coordination management for the Interdisciplinary Team. Assistance with scheduling side bars, touch bases, and risk acuity meetings. Assisting the department with managing and updating outlook schedules; care plans, and assessments. Acts as a customer service liaison with outside vendors and the Interdisciplinary Team. Assist in technology coordination (Grand Pads, Care Coaches, etc.). Assists the Interdisciplinary Team with completing the regulatory components of Service Determination Requests and Advanced Action Notices. Ensure return of care plan agreement form. Fax Care Plan's to AFC/ALF/SNF, following care plans. Scan documents to medical records. Calling hospitals/doctors' offices/etc. to obtain DPOA documents for new enrollments. Care plan Reminder Calls to participants and/or designated representative including sending Zoom link to Grandpad calendar or appropriate destination. Assist with multiphone line as needed. Coordinating Self Determination paperwork. Assist the center manager in distributing and monitoring completion of grievances. Responsible for completing all required education and file requirements on time. Keeps leader informed of problems or issues. Performs other duties as assigned. Qualifications High school diploma, or its equivalent, Associates degree preferred. Ability to work both independently and as an effective member of a team. Demonstrates initiative and organizational skills. Demonstrates interest and ability to work in interdisciplinary team environment. Strong group leadership skills. Detail oriented with good problem-solving skills. Strong communication skills, both written, and verbal. Well-developed ability to show discretion and maintain confidentiality. Advanced ability in the use of computer software, correspondence skills, report writing and telecommunications. Excellent communication skills, both written and verbal. Minimum of one year of experience working with the frail, elderly, or long-term care population. Specific Requirements Demonstrates our core values of compassion, integrity, teamwork, and excellence. Work Environment Visible presence while working in the center and is subject to frequent interruptions. Communicates with staff, participants, caregivers, and other organizations, as necessary.
