OVERVIEW Coordinates support services involving case management, outreach, assessment, service linkage, monitoring, and advocacy to infants, toddlers, and their families. Work is performed under general supervision and is guided by agency and PE-ID policies and procedures, Part C Practice Manual, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines.
DUTIES AND RESPONSIBILITIES Provides initial contact for families, completes an intake with the family, facilitates the team assessing the needs of the infant or toddler and family to determine the appropri­ateness for services and develops individual service plans in collaboration with early intervention services team. Links the individual to services and supports specified in the individual service plan to include transition from early intervention services. Assists in locating, developing or obtaining needed services and resources, including crisis services for the child and/or family. Coordinates services and benefits with the other agencies and providers involved with the individual or their family members. Assists the family in completing the financial agreement and applying for eligible benefits. Maintains necessary casework records on individuals to document the provision of support coordination for MEDICAID SPO, Waiver, and Part C reimbursement and other purposes. Provides supportive services to family/significant others. Serves on treatment teams. Advocates on behalf of the individual for appropriate treatment/interventions. Makes collateral contacts with the parents, guardians, friends, and siblings of the persons served to promote implementation of the services plan and community adjustment. Monitors service delivery to assure implementation of the individual service plan and participation in appropriate types and levels of services. Attends in-service training and staff development programs to enhance the case management services. KNOWLEDGE, SKILLS, AND ABILITIES Thorough knowledge of Parts B and C of IDEA; thorough knowledge of federal and state laws and regulations regarding infant and child services; considerable knowledge of infant/child development and associated abnormalities and treatment interventions, early intervention and transdisciplinary and interagency service delivery; thorough knowledge of community resources and treatment providers for the subject population; considerable knowledge of client family counseling techniques and practices, considerable knowledge of social casework techniques; considerable knowledge of the service planning process and related documentation; considerable knowledge of clinical documentation and office practices; ability to conduct effective interviews, observe, assess and record child behaviors and functioning and develop treatment and service plans; ability to identify needs and link individuals with appropriate services; ability to establish and maintain rapport with infants and children; ability to establish and maintain effective working relationships with agency and facility staff and of other private and public facilities, agencies and organizations; ability to communicate effectively orally and in writing; ability to operate a vehicle; ability to function calmly in stressful situa­tions; ability to prepare and maintain detailed records and prepare related reports; ability to collect, compile and evaluate electronically sourced or other data and prepare reports; ability to persistently negotiate with individuals and service providers.
TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCE Bachelor's Degree in a related Human Services field and considerable professional work experience involving case management services to intellectually disabled/developmentally delayed infants and children.
