Job Summary:University Home Care is looking for a compassionate and organized Hospice Coordinator to join our team in Livonia, Michigan as a Contractor Healthcare/Medical professional. As a vital member of our team, the Hospice Coordinator will be responsible for coordinating and providing support to our hospice patients and their families. This individual will work closely with our interdisciplinary team to ensure high-quality care and support during end-of-life care.Compensation & Benefits:The Hospice Coordinator position is an individual contributor role with a compensation range of $17 to $23 per hour, paid weekly.Responsibilities:Coordinate hospice care services for patients and their families in a timely and efficient mannerCollaborate with the interdisciplinary team to develop individualized care plans for each patientEducate patients and families on hospice services, procedures, and options for end-of-life careConduct regular assessments and evaluations of patients to ensure their physical, emotional, and spiritual needs are being metMonitor and manage patient medications, symptom management, and pain controlCommunicate effectively with physicians, nurses, caregivers, and other healthcare professionals to ensure continuity of careProvide emotional support and counseling to patients and families during end-of-life careMaintain accurate and updated patient records, ensuring compliance with HIPAA regulations and documentation requirementsParticipate in on-call duties as needed to provide support and services outside of regular business hoursStay current on hospice care trends, techniques, and regulations through continuing education and training opportunitiesRequirements:Bachelor's degree in Nursing or related field from an accredited university, or equivalent experience (NOT REQUIRED)Minimum of 2 years of experience in hospice or palliative care settingExcellent communication, organizational, and interpersonal skillsAbility to work independently and as part of a teamKnowledge of Medicare and Medicaid hospice regulationsCompassionate, empathetic, and patient-focused attitudeBasic computer skills and proficiency in using electronic medical records systems, office settingMust be able to pass a background checkEEOC Statement:University Home Care provides equal employment opportunities to all employees and applicants for employment without regard to race, color, religion, sex, national origin, age, disability, or genetics.
