UnityPoint At Home Hospice Aide- Taylor House Des Moines, IA 32 Hours/week - Tues day-Friday 3PM-11:30PM Full-time benefits **$1,500 sign on bonus for external hires** The Hospice Aide ensures quality and safe delivery of services to patients according to individualized care plan under the supervision of a Registered Nurse. Uses equipment and supplies safely, effectively, and efficiently Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served Possesses communication skills, including the ability to read, write, and verbally report clinical information to patients, caregivers, and other hospice staff. Recognizes the influence of personal, spiritual, and cultural values on perceptions regarding death and dying and their impact on quality of life .
Education: Completion of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care Aide/CNA work since completion.
Completion of a 120 hour nursing assistant course, is preferred. High School diplooma or GED, is preferred . Experience: Must be at least 18 years of age in accordance with regulatory requirements.
2-5 years hospice experience in a nursing or hospice facility, is preferred.
License(s)/Certification(s): Valid driver's license when driving any vehicle for work-related reasons.
CPR Certification.
Valid Mandatory Reporter course completion by state(s) requirement
Valid automobile insurance in accordance with state and/or organizational requirements
Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities.
Valid State of Iowa or Illinois Certification, is preferred . Knowledge/Skills/Abilities: Strong interpersonal skills
Ability to work as a collaborative team member across service lines as position requires
Ability to understand and apply guidelines, policies and procedures
Proficiency with computer applications (i.e. Excel, Word, Access), and a Smart Phone
Other: Registry/ Continuing Education Requirements: Illinois: Registration on the State of Illinois Health Care Worker Registry is required for employment in Illinois. Registration must be completed during Orientation period.
Iowa and other States: Registration with the State registry site is preferred.
