What You'll Be Paid (actual pay depends on qualifications): $25.68 – $34.74/hour
How It's Classified: Fulltime-Regular (40 hrs, Full Benefits)
The Work Schedule: Monday - Friday 8:00am - 5:00pm, occasional evening hours required

Johnson County Mental Health is looking for a qualified candidate to fill a Case Manager position on the Homeless Services team to work with adults who experience severe mental illness. The Case Manager works as part of a strengths-based case management team, providing community-based services to Johnson County residents. Some evening availability is required and the schedule will vary based on the needs of the clients served.  What You'll Do:
Conduct assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility; interpret and communicate relevant information to individuals, families, and other stakeholders.Develop plans to address the needs of individuals in single or multiple areas of focus; network with other community agencies to research and coordinate a variety of resources and services to address short term intensive or long term individual needs; encourage the establishment and maintenance of support systems; consult and collaborate with other providers in the development, implementation and review of plans.Provide a variety of services for a designated case load of individuals with multi-faceted and/or ongoing needs; ensure services provided are consistent with individual plans; initiate and attend individual and/or group meetings and provide services in a community based setting; provide transportation of individuals according to the needs identified in plans; organize and facilitate skill development and support groups.Complete and submit documentation regarding services provided; evaluates and documents individuals' progress according to measurable goals; review quality and effectiveness of services provided.What We Require:Bachelor's degree in Human Services or related fieldValid driver's license with acceptable driving record.  Experience may be substituted for degree. Education may be substituted for experience.
Johnson County Government requires reference/background screening for all positions.  Specified criteria may vary by Department/Agency.
What Also Helps (Preferred): One (1) year of relevant work experience. How Often Driving Will Be Needed: Primary (25% or more)
