Responsibilities: Conduct comprehensive patient assessments and develop individualized care plans. Administer medications, wound care, and other treatments as prescribed. Educate patients and families on disease management and self-care strategies. Collaborate with physicians, therapists, and other healthcare professionals. Maintain accurate and detailed patient documentation. Qualifications: Active and unencumbered RN license in the state of Oregon. Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) preferred. Open to candidates without direct home health experience will train. Long term care background is a plus! Strong assessment, critical thinking, and problem-solving skills. Excellent communication and interpersonal skills. Reliable transportation and a valid driver's license.
