The Healthcare Home Nursing Director/Nurse Care Manager is responsible for the supervisory oversight, training, documentation and coordination of Nurse Care Managers and is responsible for a caseload of up to 100 Healthcare Home enrollees. Provides integrated and comprehensive care management services for adults with severe and persistent mental illness, who are members of Healthcare Home. Responsible for conducting team meetings and individual supervision that promotes client and staff growth. With consultation from Physician Consultant, uses population health data to provide interventions, training and education to clients and staff. Participates in all aspects of continuous quality improvement. The Healthcare Home Nursing Director/Nurse Care Manager is responsible for maintaining a clean, safe, and therapeutic environment in compliance with all applicable licensing standards, CPRC standards and local fire and safety requirements. am. Is a member of the leadership team and advocates for integrated healthcare delivery. Exercises considerable judgment and initiative in ensuring that the integrity of services is consistent with Adapts Mission, philosophy, and service principles.
ESSENTIAL FUNCTIONS JOB SPECIFIC
Healthcare Home Nursing Director:
Develops and maintains working relationships with primary and specialty care providers, including inpatient facilities
Develops models and process to incorporate and promote integrated treatment across all services
Tracks enrollment, declines, discharges, and transfers; ensures optimal enrollment to slot capacity
Promotes the development of working relationships (including MOUs) with hospitals, and coordinates hospital admissions and discharges with NCMs
Prepares all required documentation requests and paperwork with regard to client deaths and death reviews, if applicable
Coordinates program operations with Assistant Executive Director
Complete all related tasks in conjunction with the DCCO, Healthcare Home:
o Oversees the daily operation of the HCH
o Provides leadership in the implementation and coordination of Healthcare Home activities
o Designs and develops health and wellness initiatives
o Champions Healthcare Home practice transformation
o Assists development of staff training on health issues
Complete all related tasks in conjunction with the HCH Quality Director and DCCO Healthcare Home:
o Monitors Healthcare Home performance and leading improvement efforts
o Coordinates review and utilization of Care Management reports
o Reviews and/or completes monthly implementation reports
o Participates in monthly implementation calls
o Assists in development and revision of EMR
o Actively participates in accreditation process
Lead and manage a team of Nurse Care Managers:
o Provide administrative and clinical supervision
o Ensure adequate staffing patterns to meet needs
o Work with the Nurse Trainer/Floater to orient new nurses as needed
o Disseminate information regarding policies and procedures to staff
o Conduct AIM discussions with direct report staff
o Conduct regular supervision and monthly team meetings with NCMs, and support staff opportunities to facilitate professional growth
o Ensure completion of all required nursing interventions as defined by DMH regulations as well as service provision expectations
o Ensure proper documentation of all HCH nursing services
o Serve as the nurse for the ITCD Team
o Provide backup to any direct report position as needed
Nurse Care Manager:
Responsible for completion of monthly assigned interventions, and all aspects of care according to DMH regulations, for a case load of up to 100 Healthcare Home clients
Meet with assigned clients to complete health screening initially and annually; identify risk level, care caps and make recommendations
Assist in developing a physical health goal for the Wellness Plan; promote healthy lifestyles including healthy nutrition, physical activity, and freedom from tobacco/nicotine use
Ensure that clients are referred to a PCP and/or Specialist if they do not have one; provide education to clients regarding the importance of having a PCP and encourage regular usage of their PCP for screening and management of chronic health conditions; provide education regarding the appropriate use of PCP/Urgent Care/ED
Complete Metabolic Syndrome Screenings on assigned clients following the Metabolic Screening Policy and Standing Order
Perform Medication Reconciliations for HCH enrollees who have been discharged from the hospital per agency guidelines
Provide education to clients/caregivers identified on monthly reports, and as needed, regarding physical health conditions, treatment, screening, prevention, and self-management
Provide consultation and education for community-based and office-based staff about identifiable health conditions
Consult with Specialized Healthcare Consultant as scheduled and as needed
Assist in contacting/collaborating with medical providers and other human/social service agencies regarding significant health information to ensure the provision of holistic services
Function as part of a multi-disciplinary team; attend team meetings, CPRC/SUD team huddles, and integrated meetings as assigned
Attend agency meetings and DBH/Council meetings as assigned
Travel to assigned clinics; travel to other clinics/locations for nursing coverage and trainings/meetings as needed
Travel to clients home when indicated
Meet service provision expectations per client needs and company guidelines
Associates/Bachelor"s Degree in Nursing
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
At least two (2) years of experience working with the chronically mentally ill population required
Supervisory experience is preferred
Relevant experience working in health care field preferred
LICENSURE/CERTIFICATION
Must obtain and maintain certification in CPR, First Aid, and CPI
Licensed by the State Board of Nursing as a Registered Nurse
