Responsibilities
• Develop strong relationships with patients to assist them with their care• Use data to support patients with medication adherence and best practices• Make outbound calls to follow up on outstanding needs• Use software to track progress on scheduling patients for screenings• Maintain a record of patient interactions and communicate with providers• Educate patients about healthcare options, insurance benefits, and conditions• Respond to patient questions about insurance coverage• Connect patients with community resources• Assist healthcare providers with administrative tasks• Schedule clinic visits and coordinate care for discharged patients
Requirements
• Love for the community and desire to see it thrive• Enjoy meeting new people and developing relationships• Quick learner and able to apply new information• Strong service mentality and ability to influence stakeholders• Problem-solving skills and initiative• Comfortable with change and working independently• Familiarity with smartphones and computers• Valid driver's license• Full-time availability, Monday-Friday 8am to 5pm
Key Skills Needed
