Full Time Case Manager

Job summary
The Case Manager's primary job is to enhance the overall treatment processAssist counselors in meeting the treatment needs of patientsProvide recovery support services and build business relationships

Responsibilities
• Provide guidance and assistance in accessing multiple levels of care• Facilitate patient access to substance use disorder treatment and community recovery groups• Help patients access support across multiple systems of care• Communicate with patients and develop treatment plans• Assess and track patient needs and progress• Document conversations and billing• Facilitate individual and group case management• Build relationships with referral sources• Conduct face-to-face meetings with decision makers• Collaborate with the intake team and submit reports• Maintain professionalism and positive interaction with staff• Other duties as assigned
Requirements
• Bachelor's or Master's degree in relevant field• At least 1 year of full-time employment experience• Excellent customer service and communication skills• Understanding of substance use disorder and industry trends• Knowledge of HIPAA, Federal, State, and CARF standards• Proficiency in PC skills and record-keeping• Interpersonal and organizational skills• Troubleshooting and problem-solving abilities• Self-starter and strategic thinker• Satisfactory references, drug screen, and background check
