JOB TITLE: Family Reconnect Worker Tentative hours : Monday to Friday 9:00am to 5:00pm
Job Type: Full-time Permanent Position, with Benefit Package
Salary: 44,000.00 per year
Reports to: Manager of Services
oneROOF Youth Services is a multi-service agency for youth (12-25) experiencing or at-risk of homelessness in the Waterloo Region. Our services include meeting basic needs, outreach, streets to housing support services, family reconnect support, mental health, and addictions support/services, providing harm reduction supplies, drop-in services, food hampers, and emergency shelter.
JOB SKILLS, DUTIES & RESPONSIBILITIES: • Develop Intensive Case Management Plans for youth
• Develop/adopt a Family Assessment Tool
• Assess extended family members as suitable family re-connect options
• Organize, lead, and facilitate meetings with individual family members, youth, and family group
• Develop relationships with family members and
• follow-up regularly to ensure integration/re-integration into the family unit is continuing
• Formalize discharge plans for each youth
• Increase youth community engagement
• Build self-esteem and self-efficacy in youth
• Increase life skills development
• Utilize the YAP Tool with youth and make appropriate referrals
• Link youth to necessary internal and external resources
• Support youth in finding suitable alternative housing
• Attend staff meetings, supervisory meetings, and case management meetings
• Assist youth in identifying and progressing towards their goals
• Maintain client files and statistics
• Build rapport with clients accessing drop-in
• Track and document emerging trends in youth homelessness
• Research best practices regarding programs for homeless and high-risk youth
• Ensure policies and procedures are adhered to
• Attendance/participation at oneROOF events and fundraising activities
• Public Speaking as assigned
• Other duties as assigned
EDUCATIONAL AND EMPLOYMENT ASSETS/REQUIREMENTS • A relevant degree/diploma in psychology, social work, or related discipline, as well as relevant work experience is preferred
• Experience working with at-risk youth and/or vulnerable populations.
• Knowledge of community resources and local mental health, educational, and youth justice systems and services would be beneficial.
• Acknowledge base concerning child psychopathology and ability to remain current regarding empirically based best practices.
• An ability to engage and motivate youth
• Excellent communication skills, both written and oral are required
• Ability to work with minimal supervision and in a team-oriented environment
• Must have a valid First Aid and CPR certification
• Must produce a recent Vulnerable Sector and Criminal Record Check
