Job Description Position Summary The primary responsibility of this position is to assure the delivery of the highest level of clinical care for residents of The Green House homes, to monitor regulatory compliance, and to serve as a supervisor to the Clinical Support Team while following current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities.
Essential Duties Visits each Green House home on a regular basis to monitor clinical care and status of the Elders. Make daily rounds to ensure all nursing and other Clinical Support Team Members are performing their work assignments. Plan, develop, organize, implement, evaluate, and direct the nursing services, as well as programs and activities. Develop, maintain, and periodically update written policies and procedures that govern the day-to-day functions of the nursing services. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Work with the nurses and other Clinical Support Team members to develop work assignments and schedule duty hours. Recommend the number and level of nursing personnel to be employed, to assure compliance with staffing requirements, regulations and organizational policy. Assist in calculating the number of direct nursing personnel care on duty each shift. Review complaints and grievances. Participate in the interviewing and selection of Elders for admission. Develop a written plan of care for each resident that identifies the problems/needs, indicated the care to be given, goals to be accomplished, and which professional service is responsible for each element of care. Monitor timely completion of care planning process per organizational policies.  Prepare and plan the nursing service department's budget and submit to the Administrator. Schedule, attend, and participate in Clinical Support Team Meetings. Review and assure nurse compliance with in-service requirements and programs for self-development. Demonstrate a passion for caring as evidenced by interaction with co-workers, Elders, families, and visitors. Monitor timely compliance of collection and submission of MDS data per organizational policies. Monitor effectiveness of the infection control program. Obtain regular verbal report from nurses regarding the clinical status of the elders in The Green House homes. Assure a program of staff development that supports high levels of staff skill. Monitor QIs and coordinate a quality improvement program with the individual house Clinical Support Team and Shahbazim Review Elder clinical status as required, using visits and clinical reports from organizational tracking systems. Discuss observations/concerns about clinical compliance with the CST nurses, and/or Shahbaz Care Coordinator as applicable based on the situation. Discuss observations/concerns about housekeeping issues with the Shahbaz Housekeeping Coordinator. Discuss observations/concerns about dietary issues with the Shahbaz Food Coordinator. Provide education to nurses and other Clinical Support Team Members to improve their skills. Maintains a good relationship with the Medical Director and works with him/her to ensure the highest quality of care. Knows and maintains compliance with all state and federal regulatory requirements. Maintains and supports a culture of compliance, ethics and integrity. Maintain confidentiality and rights of resident, family, employee, and facility information in compliance with PHI HIPAA policies and applicable regulations. Maintains safe work environment and exhibits safe work practices. Perform all other duties as assigned. Qualifications $105,000 to $113,000 Annually
Basic Qualifications & Experience Must possess, as a minimum, a Registered Nursing Degree from an accredited college or university. Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility. Must have, as a minimum (6) months experience in rehabilitative and restorative nursing practices. Exhibits a high level of leadership ability. Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state. Must possess a valid and unrestricted Driver's License. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. Must be able to read, write, speak, and understand the English language. Must possess excellent verbal and written communication skills. Must possess the ability to deal tactfully when personnel, residents, family members, visitors, government agencies/personnel and the general public. Must possess the ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. Must be able to work in a non-hierarchical, interactive, and coaching philosophy with hands-on care partners. Must be able to relate information concerning a resident's condition. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. Must have patience, tact, a cheerful disposition and enthusiasm, as well as willingness to handle challenging people, situations, and interpersonal relationships. Must not pose a direct threat to the health or safety of other individuals in the workplace. Must maintain the highest levels of confidentiality of both resident and organizational information. Working Conditions / Physical Requirements Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility. Must be able to push, pull, move, and /or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet. May be necessary to assist in the evacuation of residents during emergency situations. Moves intermittently during working hours. Walks long distances between houses and the administration building. Is subject to frequent interruptions. Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.  Is subject to hostile and emotionally upset residents, family members, etc. Works beyond normal duty hours, on weekends, and in other positions temporarily, when necessary. Must be available by phone or email unless otherwise relieved from that duty. Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.). Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants. Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses. May be subject to the handling of and exposure to hazardous chemicals.
