­­­­­­­­­­­­­­­­­­­­­­­­­­­­­­­­­­Announcement # 2024-114 Diabetes Educator-RDN Nutrition Program Department of Tribal Health Hourly Wage: $39.82/Regular/Full-Time Location: Yakama, Indian Health Services This position will have a high level of responsibility working with individuals who either already have diabetes or are at high risk for developing diabetes. Provides Medical Nutrition Therapy (MNT) and diabetes education to those served by the Yakama Indian Health Center. Assesses, evaluates, educates, and helps patients formulate an education plan based on national standards for diabetes self-management. Works with individuals to identify and encourage opportunities for lifestyle and dietary changes. Diabetic foot exams, assistance with CGM placement/education, and blood glucose meter training, are also required.
Examples of Work Performed: Provides and documents MNT and diabetes education services. Counsel clients or patients, individually or in group sessions, to identify and encourage opportunities for lifestyle changes. Facilitates referral process to assure continuity of care with IHS primary care and Healthy Heart providers. Coordinates with other federal or tribal programs, agencies, and community resources in order to serve as a clinical resource for diabetes and nutrition care. Follows all related Federal, State, local and agency regulations for Registered Dietitian and Certified Diabetes Educator services. Maintain professional growth and development through seminars, workshops, and professional affiliations to keep abreast of the latest health trends. Assists Nutrition Manager in evaluating program services and formulating goals and objectives to promote the health of the community. Reviews patient education materials. Develops new diabetes specific materials as needed. Other duties as assigned. Knowledge, Skills and Abilities: Knowledge of basic principles, practices, and professional standards of providing nutrition and diabetes education services to tribal communities. Knowledge of the Yakama Nation Personnel Policies. Knowledge of documenting in patient medical files, HIPAA laws, medical terminology and EHR standards. Knowledge and proficiency in computer use. Ability to maintain comprehensive detailed reports and correspondence with completed documentation in adherence with State, Federal and Tribal policy for billing purposes. Ability to establish and maintain effective working relationships with other department staff, supervisors/managers, and the public. Ability to encourage a positive attitude toward good nutrition and lifestyle choices. Minimum Requirements: Bachelor's degree in Nutrition or Dietetics. Must be licensed as a Registered Dietitian with the State of Washington. Current credentials as a Certified Diabetes Care and Education Specialist. Three years of related experience working with pre-diabetic or diabetic individuals. Must pass a pre-employment background check. Required to pass a pre-employment drug test. Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving Permit. Enrolled Yakama Preference, but all qualified applicants are encouraged to apply.
