Responsibilities
• Perform physical and environmental assessments based on protocols• Provide non-emergency clinical care with a focus on chronic disease management• Work as an extension of virtual care teams• Respond to crisis calls and provide clinical care• Conduct environmental assessments in support of the patient's plan of care• Provide ongoing interaction with patients through calls and home visits• Provide disease-specific education to patients and caregivers• Communicate with multiple agencies and members of the Acadian Health team• Maintain patient medical records• Participate in activities that promote Acadian Companies
Requirements
• Current BLS and ACLS Certifications; PHTLS and PALS Certifications preferred• Required licensure as a Paramedic within the state of employment• Ability to obtain Community Paramedicine (CP-C) certification or licensure within one year• Effective communication skills• Coaching and leadership abilities• Teaching and educational facilitation skills• Familiarity with basic computer applications• Team-oriented and able to work effectively with others• Valid driver's license and acceptable driving record• Recommendation and minimum of two years direct patient care experience (for current employees)• Degree from an accredited college or university preferred• Previous trainer or instructor experience preferred• Previous Critical Care experience preferred
