CLINICAL PHARMACIST SITE OF SERVICE :  Naval Hospital Bremerton, WA Full Time POSITION QUALIFICATION/REQUIREMENTS: Degree : A Baccalaureate Degree in Pharmacy from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE); OR A Doctorate degree in Pharmacy (PharmD) from a College of Pharmacy accredited by the American Council for Pharmacy Education (ACPE). Certifications : Current Basic Life Support (BLS) hands on certification through the American Heart Association or American Red Cross. Experience : A graduate certificate within the preceding year from an accredited pharmacy residency program OR Possess a minimum of 12 months of experience within the preceding 18 months as a Clinical Pharmacist performing direct patient care clinical activities in an inpatient, outpatient, or ambulatory care pharmacy setting OR Have held clinical privileges as a Clinical Pharmacist within the preceding 36 months in a military treatment facility Internship /Residency :  Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training and/or CME in the practice of clinical pharmacy. Licensure : Possess a current and unrestricted license to practice as a Pharmacist in any one of the 50 states, the District of Columbia, Puerto Rico or the U.S. Virgin Island. U.S. Citizenship : Shall be a U.S. citizen English Language Requirement: Able to read, write, and speak English well enough to effectively communicate. Physical Capability: Shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required under the contract. HOURS: The HCWs shall provide services Sunday through Saturday, including the day of observance of Federal holidays between the hours of 0700 and 0659.   The HCW shall not be scheduled more than a 12.75-hour shift (including a 45 minute uncompensated meal break) in a 24 hour period.  The HCWs shall be scheduled no more than 4 holiday shifts. The HCW shall not exceed 80 hours of service in a two-week period. BENEFITS: Medical/Dental/Vision/401K Life Insurance/Disability Insurance Continuing Medical Education Reimbursement SPECIFIC DUTIES:
Provide medication information to the clinical staff as required, including in-service training on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head. This is not a medication management role. Maintain inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security.  Checking and dispensing of new prescriptions, refilling prescriptions, entering orders into a database, monitoring for drug interactions/allergies and monitoring drug overlaps. Providing services as a drug information consultant to support new and refill prescriptions, patient requests, reporting allergies, reviewing patient profiles, and lab values. Monitor and perform distributive functions for inpatient and outpatient pharmaceutical services including work units for outpatient prescription preparation and dispensing, unit dose, sterile products, bulk orders, and parenteral nutrition and chemotherapy. Meets with patients to: Obtain a complete drug history. Monitor the patient's response to drug therapy through all available sources, such as, patient and pharmacy records, and laboratory data. Educate and counsel the patient regarding his/her drug therapy and/or protocol. Explain information about current drug therapy and the potential side effects. SPECIAL REQUIREMENTS/SKILLS Must be comfortable in a fast-paced, dynamic environment.  Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects.  Flexible and able to work with various personalities.  Team-work skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
