Responsibilities
• Conduct comprehensive assessments of referred member's needs/eligibility• Inform the case management process to facilitate appropriate healthcare outcomes• Utilize clinical tools and information/data review• Evaluate member's benefit plan and available programs/services• Collaborate with case management team• Coordinate care for members• Support and educate members• Implement Health Services strategies and programs• Utilize care management tools and resources• Travel to meet members face to face
Requirements
• 3 years of direct clinical practice experience post masters degree• 1 year of Case management experience• Unencumbered Behavioral Health clinical KS state license• Willing and able to travel up to 50% of their time• Reliable transportation required• Willing to work beyond core working hours• Discharge planning experience preferred• Crisis intervention skills preferred• Managed care experience preferred• Hospital experience preferred
