Department Administrative/Clerical Support
• Answers telephone: screens calls and callers, referring them to appropriate associate/department
• Maintains designated filing and record keeping systems. Assists with preparation of patient lists and reports.
• Performs other related secretarial and clerical functions as requested
Care Coordination Duties
• Collaborates with Integrated Care Management team members with discharge planning activities.
• Clearly communicates with team members and key stakeholders in a timely manner to support timely discharge from the acute level of care.
• Arranges for patient transport to the next site of care. Includes but not limited to wheelchair vans, ambulance and other modes of transport as needed.
• Follow up on cases with Medicaid pending on a weekly basis and update leadership and team members in a timely manner.
• Obtains authorization information for admissions to Skilled Nursing Facilities or Inpatient Rehab Facilities
• Completes on-line E-filing updates with the Iowa Courts as needed
• Facilitates FMLA paperwork completion to support patients and families.
• Performs Notary Public duties as needed.
Regulatory Compliance
• Distributes notification letters to, provides education to, and secures signatures from patients per CMS regulations surrounding admission status and right to appeal discharge.
• Audits compliance
Education: High School Graduate
Associates degree preferred
Experience: Healthcare experience preferred
License(s)/Certification(s): Valid driver's license when driving any vehicle for work-related reasons. Basic Life Support preferred.
Knowledge/Skills/Abilities: • Interpersonal skills to deal effectively and professionally with a wide range of people.
• Strong oral and written communication skills to represent the organization in an effective and professional manner.
• Keen attention to detail.
• Uses judgement and decision-making to drive performance.
• Strong computer knowledge of Windows based programs and Microsoft Office.
